Financial Aid Forms
Submitting Documents
Students may submit the majority of the forms below via email to the UDM Scholarship & Financial Aid Office or to their Financial Aid team member. However, documents containing sensitive data (anything with social security number, tax documents, citizenship documents, etc.) should not be emailed. These items can be brought in person to our office or submitted securely through the Document Submission Form.

- Special Circumstances Form and Checklist
- Statement-of-Educational-Purpose
- Summer 2026 Financial Aid Application - Students should submit this form if they would like to receive federal financial aid during the summer term. Federal financial aid includes federal grants, federal work-study, and federal student loans.
- Summer 2026 Federal Work Study Application - Students should submit this form if they are not enrolling in the Summer 2026 term but are interested in a federal work-study position during the summer term. Coming January 2026
- Untaxed-Income-Verification
- Verification Worksheet - Dependent students will need parents to complete a portion of the verification worksheet. Independent students will complete this with themselves and their spouse, if applicable.
