- Verification Worksheet - Dependent students will need parents to complete a portion of the verification worksheet. Independent students will complete this with themselves and their spouse, if applicable.

Submitting Documents
Students may submit the majority of the forms above via email to the UDM Scholarship & Financial Aid Office or to their Financial Aid team member. However, documents containing sensitive data (anything with social security number, tax documents, citizenship documents, etc.) should not be emailed. These items can be brought in person to our office or through the Document Submission Form
