Employee Benefits Plan and Summary Plan Description
Plan Amendment Summary Material Modification – COVID 19
COVID-19 At-Home/OTC Diagnostic Tests: Federal regulators recently issued regulatory guidance regarding mandatory coverage of at-home or over-the-counter (OTC) COVID-19 diagnostic tests. Detroit Mercy’s health plan will provide direct coverage, provided by the pharmacy benefit manager (PBM) at point of sale and via claims reimbursement. Plan participants also have the option for direct-to-consumer shipping program (mail order). Participants can order test via the government’s website. Any claims submitted under the medical plan will be denied by Trustmark, as claims must be submitted under the prescription plan. For OTC COVID-19 tests, plan participants are limited to 8 tests per 30 days or calendar month. Under the plan out-of-network reimbursement is limited to $12 per test. Please save all receipts for any COVID testing kits, as they will be required for reimbursement.
