UDM Student Health Center Mission

The Student Health Center is dedicated to assisting students achieve their academic goals by promoting and maintaining their physical, spiritual, and mental well being. Our services and programs educate studentsso they canestablish life-long patterns of health optimizing behaviors and prevent illness.

The Health Care Providers at the UDM Student Health Center are nationally certified Nurse Practitioners. They work in conjunction with a Physician from Providence Hospital.

• Student Health Insurance Plan Waiver / Enrollment
• Medical History Form

Please note: Commuter Students are welcome to enroll in the plan, but do not need to waive the Student Health Insurance. Only Residential Students need to waive the Student Health Insurance each Fall if they have other coverage.
