TB Form and Insurance Information

TB questionnaire

University of Detroit Mercy requires that a tuberculosis (TB) skin test report be on file for all new international students.

New incoming freshman students must complete the online TB Questionnaire and have a TB skin test if indicated by the questionnaire. The test must have been performed in the United States or Canada within the last 12 months (one year).

Student Health Insurance

All students who will be living in the residence halls must have documentation of health insurance.

Health Programming Request

This form is used by students and employees at the University of Detroit Mercy to request an educational health program from the Wellness Center.  We are best able to make your programming request if it is submitted as far in advance as possible.  We request at least a 30 days' notice from you.  Our normal business hours are between 8:30 am and 5:00 pm Monday through Friday.  Please give the Wellness Center 2 business days to get back to you with our answer on whether or not we can participate.  https://forms.office.come/r/ZznpgJ2hud
