Service-Learning Forms
- Service Learning Guide for Students: A guide for students who are new to the Service Learning program (The cover sheet is handed out during the LDI presentation)
- Service Learning Letter of Introduction to Agencies: A guide to give to the person you contact at an agency that is not on our list, or would like more information
- SL1 Student Information: This form is administered and collected when LDI does the Service-Learning presentation - if you missed it, fill this out and get it to LDI in Briggs 232. pdf version
- SL2 Service Agreement is optional. If your teacher asked you to use it, here it is. Many teachers simply use a roster that is given to your professor by LDI. DUE BY THE 4TH WEEK! pdf version

- SL4 Student post-course self-assessment is a form normally administered by LDI at one of the last classes of your course. Check with your teacher before filling this out yourself! pdf version
