Service-Learning Forms
- Service Learning Guide for Students: A guide for students who are new to the Service Learning program (The cover sheet is handed out during the LDI presentation)
- Service Learning Letter of Introduction to Agencies: A guide to give to the person you contact at an agency that is not on our list, or would like more information
- SL1 Student Information: This form confirms the type of service you are doing: DUE BY THE END OF THE 4TH WEEK OF THE TERM!
- SL2 Service Agreement is a roster that is given to your professor by LDI. There is no longer an individual form.
- SL3 Service Verification: This form you give to your service agency rep. to verify your service hours: DUE BY THE END OF THE 12TH WEEK OF THE TERM!
- SL4 Student post-course self-assessment is a form normally administered by LDI at one of the last classes of your course. Check with your teacher before filling this out yourself!
